As a free user, you’re able to request records from one provider. As a Plus user, you can request records from a maximum of three providers per month. 

Requests that have been sent will display as connected, while unsigned requests will remain in your queue until you've signed them. Select the unsigned requests to complete the process and your requests will be on their way!

Once you’ve made a request, it will be visible under the “Health providers” tab at the bottom of your screen. If your request was recently submitted, the subheading “we got your request” will appear under the requested provider's name. Your “Health providers” tab will also show all requests you’ve made to date. To see a detailed status of your request, click “See status” on the provider's tile.
