When signing up for your account, we will need you to provide us with your legal name, date of birth and email.
We need this information as a form of verification to ensure only you have access to your account and health information.
Before you can make a request, you’ll need to provide us with a photo of government issued ID along with a selfie to verify your ID. We’ll also need your address in order to add providers to your account, your phone number, and your health card number. Health institutions also use this information to confirm your identity and to find your files when you make a record request.
Please let us know in advance if your name might be different than your legal name and we can take that into account when verifying your identity.
