What kind of providers can I access records from?

You can request a medical record from any healthcare provider! The most common sources of medical records our users find helpful are hospitals, walk-in or primary care clinics, family health teams, and medical centres. If you don’t see your provider on our list, you can also add them to our database and go ahead with making a request. Do so by clicking on the purple ‘+’ button on the bottom of the home page screen of Dot Health, then clicking on ‘Request records from a provider’. Scroll to the bottom of the screen and click on ‘Other’ to add your provider to our list. 

If you would like to request a medical record from a laboratory, please request from the provider that referred you for lab work. That is where your lab results will be held. More about that here.
